How PDGM in 2026 Changed the Financial Stakes of OASIS Documentation

The shift from the old volume-based Prospective Payment System to PDGM was the biggest reimbursement change home health had seen in a generation. Under the old model, agencies got paid largely based on how many services they delivered. Under PDGM, they get paid based on who the patient is, specifically how sick and how functionally limited the patient is at the start of care.

That distinction sounds straightforward. In practice, it means the OASIS your clinician completes at the bedside on day one is doing most of the financial work for the entire episode. Get it right and you get paid what the patient's condition actually warrants. Admission source and timing are largely outside clinical control. Clinical grouping follows the primary diagnosis. That leaves functional impairment and comorbidity adjustment as the two variables where OASIS accuracy at the bedside has the most direct impact on what your agency gets paid.

The functional impairment problem

Functional impairment level is calculated automatically from eight specific OASIS items. CMS runs a points algorithm across these eight fields and assigns the patient to a low, medium, or high tier. That tier is one of the primary determinants of case-mix weight, meaning it directly adjusts the $2,038.22 base rate up or down for every 30-day period.

Here is where the timing of documentation matters most. When a clinician conducts a start of care visit at 10am and completes the OASIS at 8pm, they are working from memory. And memory of functional observation, how much assistance the patient needed to transfer from bed to chair or how unsteady they were crossing the carpet, degrades fast.

Under PDGM, a documentation recall gap is a revenue gap.

What happens in practice is what you might call regressive rounding: memory naturally smooths over detail and defaults to the more moderate answer. A patient who needed maximum human assistance during the visit gets documented as needing only verbal cues. Two or three M-items scored that way can move a patient from a high to a medium or low functional impairment tier. Because point thresholds vary by clinical group, the revenue impact of that shift can run into hundreds of dollars per episode, and it happens silently, across your entire census, with no alert.

Why QA can't fix what wasn't captured

Comorbidities: the revenue hiding in casual conversation

PDGM includes a secondary comorbidity framework that provides upward payment adjustments for cases with complex interacting conditions. These adjustments are tiered: none, low, or high. They can meaningfully increase case-mix weight when captured correctly.

The catch is that secondary diagnoses often surface organically during the visit, like a patient mentioning peripheral neuropathy while talking about their medications, or brings up a recent hospitalization while describing their daily routine. These details are clinically significant and financially relevant. But they are also easy to miss when the clinician is focused on the visit and not documenting in real time. By the end of a long day, the connection between what a patient said and what should be coded is frequently lost.

The question your agency should be asking is not whether your OASIS forms pass technical validation. They probably do. The question is whether they accurately reflect the clinical complexity of your patients at the moment they were assessed.
